If your controller develops a defect, complete this form carefully and send it to our Service Department
together with the defective controller and sensor, a copy of the bill of purchase, and the confirmation of
installation from your electrician. Without these items we will be unable to deal with your complaint!

If the defect is covered by the warranty, a replacement unit and sensor will be delivered at once.
If the defect is not the fault of the manufacturer or is not covered by the warranty, the customer will be
sent a quotation for the repair.
