When requesting warranty service, please have the following information available.
Outside the warranty period
If service is requested outside of the warranty period, the cost of service is responsibility of the product
owner. Service charges include a technical service fee, travel expenses, and parts costs.

1. Your contact information (e.g. name, address, phone)
2. Serial Number
(See the caution label on the back side of the seat lid.)
3. Date of installation
(See the warranty card.)
4. The reason for requesting service
