Medical Grade Multiple Portable Socket-Outlets conform to and shall be installed and properly grounded in compliance with requirements
of the current National Electrical Code, Canadian Electrical Code and/or codes administered by local authorities.
WARNING:
1. Only use this device for supplying power to equipment which is intended to form part of the medical system.
2. This device must be permanently attached to the medical equipment by a means such that it is only removable with the use of a tool. It shall not
be located or positioned on the floor.
3. The use of this device does not ensure that a medical system compliant with any standard or specifications (such as local codes) will result.
Only those qualified to assemble a medical system shall attempt to do so. The Standard for Medical Electrical Equipment– Part1-1: General
Requirements for Safety–Collateral Standard: Safety Requirements for Medical Electrical Systems, IEC60601-1-1 is used to determine the
suitability of a medical electrical system. In accordance with IEC60601-1-1 in a medical system the medical equipment connections to the
SPRPT shall only be accessible with the use of a tool. If you are not familiar with this standard and/ or medical systems do not use this device.
4. This device must be used within its marked load rating.
5. To disconnect this device from the mains, disconnect the plug from the mains socket.
6. Compliance with the above is checked by inspection.
CAUTION:
All electrical products may represent a possible shock or fire hazard if improperly installed or used. Do not plug into another relocatable power tap. The
3-prong, grounding type plugs in all units are for use with 3-prong, grounded receptacles only. Modifying or bypassing the ground pin for use in a non-
grounded receptacle or plugging into another relocatable power tap may lead to electrical shock, is not recommended and voids the product warranty.
All units are designed for interior use in dry locations only. Check all local and national electrical codes for proper installation.
INSTALLATION GUIDELINES:
1. This product incorporates over-current releasing only in the ungrounded phase conductor. This product must not be used in countries other than
the United States and Canada.
2. This product must be plugged into a designated wall outlet and designated wall outlets shall not be used for other equipment.
3. Grounding reliability can only be achieved when power supply is connected to an equivalent receptacle marked "Hospital Only" or "Hospital Grade".
4. These products shall not be used in the presence of flammable anesthetic.
5. These products are cord connected multiple outlet devices for use with medical electrical equipment or a medical electrical system. The product
shall be evaluated in combination with the end-use medical equipment/system in accordance with NFPA 99, UL 60601-1, CAN/CSA C22.2 No.
601.1, and/or IEC 60601-1-1 as appropriate.
6. The ULM4-x is suitable for use with Emergency Trolleys (also known as "Crash Carts") in accordance with clause 57.2e of UL60601-1.
7. For compliance with NFPA 99, Healthcare Facilities Code, a summary of the requirements is shown here but the latest edition of NFPA99 shall be
consulted to ensure compliance.
a. The multiple outlet device shall be permanently mounted to a rack, table, pedestal or cart.
b. The sum of the ampacity of all equipment connected to the multiple outlet device shall not exceed 75% of the rating of the power supply cord.
For units with a 15A attachment plug, the load shall not exceed 11.25A. For units with a 20A attachment plug, the load shall not exceed 15A.
c. The electrical and mechanical integrity of the multiple outlet device, especially the grounding path, shall be regularly verified and documented.
d. The leakage current of the multiple outlet device and connected equipment must be checked to confirm it is within acceptable levels.
e. Means shall be employed to ensure that additional devices or non-medical equipment cannot be connected to the multiple outlet device after
leakage currents have been verified as safe.
