6. KLOCKHOOF DIAGNOSIS (Modified by Lavoro Clinic in Milan)
This diagnosis evaluates the 500, 1000, 2000 and 4000 Hz frequencies. The following criteria apply in accordance with Table 6:

NORMAL The threshold does not exceed 25 dB at any frequency.
ISOLATED
ACOUSTIC No conversational loss
TRAUMA
GLOBAL
Conversational loss
ACOUSTIC TRAUMA
OTHER LOSS not due to exposure to sound.

The threshold at the 4000 frequency is below 55 dB and below 25 dB in the
other frequencies.
The threshold at the 4000 frequency is above 55 dB and below 25 dB at the
other frequencies.
One or more frequencies are not affected (are below 25 dB).
All frequencies are affected (above 25 dB) but none is above 55 dB.
All frequencies are affected (above 25 dB) and one above 55 dB.
