8. Troubleshooting
If Form is not working properly, please check the following:
Problem
The display will not
turn on
Stimulator will not
connect to the electrode
Stimulator will not
connect to the charging
cradle
Electrode won't stick to
the body
Unusually low or
inconsistent sensation

Flat battery
Damaged stimulator
Stimulator is in the
wrong orientation
Stimulator is in the
wrong orientation
Hydrogel pads are
old and have lost
their adhesion
Incorrect electrode
placement
Hydrogel pads are

Solution
Charge battery.
Contact supplier.
Rotate the stimulator so the shapes on
the bottom of the stimulator and top of
the electrode align. This should allow the
magnets in the stimulator and electrode
to attract. See section 4.
Rotate the stimulator so the shapes on

the cradle align. This should allow the
magnets in the stimulator and charging
cradle to attract.
Purchase replacement hydrogel pads
Place electrodes as per the electrode
placement guide
