3. Endogenous gases.
4. Cotton wool or oxygen-saturated gauze.
5. Flammable substances, such as alcohol-based tinctures used in patient preparation.
6. Natural flammable gases likely to accumulate in cavities like the intestine.
7. Adhesive products like flammable solvents.
Whenever possible, use a non-flammable agent for cleaning and disinfection, or let flammable
products evaporate before a high-frequency surgery.
 Dangerous voltage levels. This equipment must be used only by qualified personnel.
 This equipment produces physiological effects.
 This equipment produces interferences that could adversely influence the operation of another
electronic device.
 A defect in the equipment may result in an undesired increase of output power.
4.6.2 – Recommendations and cautions in surgeries
 Check all accessories and connections to the electrosurgical unit before starting the surgery. Make sure
that accessories are working properly. An inadequate connection may produce sparks from metal to
metal, thus resulting in a neuromuscular stimulation in the patient, malfunction of accessory and
undesirable surgical effects.
 Electrosurgery must be used cautiously in patient using an internal or external pacemaker. Interference
caused by electrosurgical current may lead to an inadequate peacemaker operation. For more
information, refer to a cardiologist or pacemaker manufacturer.
 The electrosurgical unit must be cautiously used in patients using pacemaker because interferences
may be produced in that device and more sensitive patients may feel some neuromuscular stimulation,
especially in spray coagulation and when high levels of power are being used.
 A patient should not be allowed to make a direct contact with grounded metallic objects (surgical bed,
instrumentation tables, supports, etc.) during electrosurgery, to prevent burns caused by current
deviations. When that is not possible, one must work cautiously, focused on the patient's safety. Use of
antistatic enclosures is recommended in that case.
 Contact between body parts may result in burns caused by the circulation of electrosurgical current
across such parts. They must be separated from each other by dry gauze.
 In cases where a defect or malfunction of electrosurgical unit may cause surgery interruption, having
another device in conditions to replace it is recommended.
 Care should be taken when using electrosurgery very close to or in direct contact with metallic objects,
such as forceps, specula, clamps, etc. Using electrosurgery under such conditions may cause tissue
destruction and unintentional burns.
 Use minimum power levels that are adequate to surgical procedure at issue. If the surgeon has no
personal experience about the power level to be used, it is recommended to start with a very low level,
for example 1, and increase it carefully until the desired electrosurgical effect is obtained.
 Never increase the power level without checking carefully beforehand the conditions of pencil, patient
plate cable and their respective connections. The need of a very high increase of power may indicate
problems in cables or their connections. Use the active electrode for the minimum time necessary to
obtain the surgical effect desired, to reduce the possibility of burns.
 During the surgical practice where high-frequency current could circulate through body parts with a
relatively small sectional area, use of bipolar techniques is recommended to prevent an undesired
coagulation.
 Electrode surgery cables should be set in a way to prevent contact with the patient or other conductors.
 It is recommended to aspirate the smoke produced during surgical procedures, to give the surgeon
and his team a better visibility of surgical area and increase the level of safety against contamination.
