4.1 Product Return

For the quickest response to your service needs, please follow these procedures:
Step 1: Write down Bovie
Step 2: Call Customer/Technical Services: 800.251.3000 (United States) or
+1 615.964.5532 (International) and describe the problem.
Step 3: If the problem cannot be resolved over the phone and the Bovie
Evacuator must be returned for repair, a Return Material Authorization (RMA) number must
be obtained from Customer Service before returning the Bovie
Evacuator.
Step 4: Use the original packaging material to return your Bovie
Evacuator whenever possible. If you do not have the original packaging material, ask Customer
Service for advice on how to pack for return shipment.
Step 5: Freight for all returned products should be prepaid by the facility returning the product.
A Ship To address will be supplied by Customer Service.

To reorder, obtain replacement parts, or to return Bovie
Customer Service or contact your authorized Distributor/Sales Representative.
