Note the marker that is closest to the entrance of the TE puncture.
Read the size number on the Dilator-Sizer and select a Blom-
Singer voice prosthesis of corresponding size. If the measurement
is between two markers, select the longer one.
Fully insert the Dilator-Sizer and fasten the neck strap with
adhesive tape on to the patient's neck. Leave the Dilator-Sizer in
the puncture to dilate 5 minutes, or longer if necessary, in order
to allow for easy insertion of the selected voice prosthesis. Prior
to insertion of the Blom-Singer voice prosthesis, remove the
tape from the neck strap, grasp the dilator securely, and carefully
withdraw it from the puncture. Immediately insert the voice
prosthesis.
Voice Prosthesis Sizer
The Voice Prosthesis Sizer (diagram 2) is inserted by the Clinician
into the TE puncture until it touches the posterior esophageal
wall, directed down the esophagus, and gradually withdrawn
up and out of the puncture until very gentle resistance of the
esophageal collar (diagram 2a) against the anterior esophageal
wall is detected. The sizing marker visualized nearest to
but outside the puncture site at the posterior tracheal wall
determines prosthesis length. Note: Pulling too firmly on the
Voice Prosthesis Sizer may squeeze or compress the TE tract
resulting in an erroneous measurement that indicates it is shorter
than it actually is.
1. Place the tip of the inserter into the open end of the Voice
Prosthesis Sizer (diagram 7). Hold the Voice Prosthesis Sizer by the
neck strap (diagram 2b) to avoid touching the portion that inserts
into the TE puncture.
2. Attach the neck strap on the Voice Prosthesis Sizer securely
over the safety peg (diagram 7a) on the inserter (diagram 8). This
should prevent inadvertent dislodgment of the device during
insertion into the TE puncture.
3. Hold the end of the inserter (diagram 9) and insert the Voice
Prosthesis Sizer completely into the TE puncture. Next, carefully
withdraw the Voice Prosthesis Sizer until very gentle resistance is
detected indicating that the esophageal collar is seated against
the anterior wall of the esophagus (diagram 11).
